That is worth more than ranking in Largo alone, because St. Petersburg is the commercial center of the area and people in the suburbs around it search toward it. The map pack is not the only way into a city, and it is the only part of a city that is closed to you.
Why patients in small suburbs search for the big city next door
When somebody in a small city needs a specialist, they often do not search their own town. They search the nearest city they think of as the place where things are. Largo residents search St. Petersburg, and the same thing happens in every metro we work in. Suburban patients attach the name of the big city they already drive to, not the name of the town they live in.
Most practices never try to rank in the bigger city because they assume they cannot, since they are not located there. The map pack is indeed closed to them. The organic results are not. Those are won by the page, not by the pin on the map.
What we built for them
A dedicated landing page for each condition in each city they can realistically serve. Back pain, neck pain, sciatica, knee pain, hip pain, shoulder pain, arthritis, neuropathy, and the procedure pages for PRP, shockwave therapy, regenerative treatment and bone marrow aspirate concentrate. Each one exists for St. Petersburg, for Largo, and for Clearwater.
We also optimized the Google Business Profile for Largo, where they can win the map, and kept the two efforts separate on purpose. Map pack in the city you are in. Organic in the city you want.
Where they rank today
Live, from a searcher in the Tampa Bay area, they hold the number one organic result for "pain management st petersburg." In Largo they are number two organic and number one in the map pack. For "pain management clinic st petersburg," they sit in the top five organic and Google's AI Overview cites them in its answer.
Averaged across 90 days in Google Search Console:
- "neck pain treatment st petersburg fl": position 3.4
- "pain management doctors st petersburg fl": position 3.7
- "sciatica treatment st petersburg fl": position 5.2
- "pain management seminole fl": position 5.2
- "back pain treatment st petersburg fl": position 5.5
- "pain management largo": position 5.6
Those are 90 day averages from Google Search Console, pulled in October 2026. We report averages rather than a single day's screenshot because local rankings move with where the searcher is standing, and an average across 90 days is harder to cherry pick.
Across the last 90 days the site ranked in the top 100 for more than 900 non branded searches, which is where our reporting view tops out, so the real figure is higher. Of those, 215 sit in the top 10 and 89 in the top 5, and 126 of the top 10 placements are local searches tied to a city.
The site's average position across everything improved from about 22 to about 13 over fourteen months, and their best month for clicks was the most recent full one, which is what we look for. The trend line still pointing up after a year is the thing that matters, not any single month.
What happened in ChatGPT
Asked who the best pain management clinic in Largo is, ChatGPT returns this practice first and calls it "my first choice to investigate," ahead of two other named clinics.
Asked who the best pain management doctor in St. Petersburg is, ChatGPT names three doctors, and this practice's owner is the second of the three. Again, the practice is not in St. Petersburg. The model picked it up the same way Google did, from pages that say plainly what this practice treats and which areas it serves.
We check these the way a patient would: logged out, no account, no chat history.
Why this practice is a harder case than it looks
Pain management is a crowded field with hospital systems, multi location groups and national directories competing for the same searches. Healthgrades and Yelp sit on page one for almost every term. Ranking a single practice above a hospital network on its own city name is not a quick win, and it did not happen quickly here. It took sustained publishing across conditions and cities.
That is also why the honest version of this case study leads with the position improvements and the ChatGPT placements rather than a dramatic traffic chart. The traffic climbed steadily rather than exploding, and the practice gets a large share of its searches from people looking for the doctor by name, which is what happens when a practice has been established in an area for a long time.
What we would do the same for another practice outside the city it wants
Win your own city first, in both the map and the organic results, because that is the foundation. Then build the condition pages for the bigger city next door and accept that the map pack there is not available to you. Target the specific condition plus the big city name, where a dedicated page can beat a hospital's generic department page. Do not claim an address you do not have.
What practices ask us about this account
What does this cost and what do I have to do?
The $97 test covers one keyword and four pages, so you can see the work before you commit to anything. Ongoing work is a monthly retainer starting at $495 and we quote it after we look at your market, because one city costs less than a cluster of them. Your side of it is approving the keyword list and correcting us when we get something clinical wrong. We write the pages, build them and publish them.
Can any practice rank in a city it is not located in?
In the organic results, often yes, with the right pages. In the map pack, no. Google places businesses in local map results by their actual address, and no page can change that.
Does the distance hurt the rankings?
It does. The further you are from the searcher, the harder the organic placement, and a large metro name is usually the last thing to come. That is why we build outward from the home city instead of starting with the biggest one.
How long before this shows up?
Individual pages can rank in days or weeks. Beating hospital systems on a city name is a longer project, measured in months of steady publishing.
Which practice is this?
The whole advantage on this page is that their competitors in the larger city have not worked out where the traffic is going. It is a pain management practice based in Largo, Florida.
